Janice E. Glick, LICSW, C-ASWCM
Professional Geriatric Care Manager

Janice Glick, LICSW, C-ASWCM, has been Director of Social Work Services Department at the Jewish Rehabilitation Center for the past 12 years with a focus in geriatric care management. She has practiced geriatric care management in hospital settings, and skilled nursing facilities She has also had a private practice in Salem as a clinical social worker and worked with individuals and families in several out-patient mental health clinics.

In addition, Janice is a trained mediator. She has completed mediation training in M.G.L. ch.233 23C at Mediation Works Incorporated. She has also participated in advanced elder care mediation program training. Janice serves as a field supervisor for Graduate Schools of Social Work Programs in the Boston area and is an adjunct faculty member in the Social Work Program at Salem State College. She has delivered various workshops locally as well as nationally regarding elder care issues.

Janice received her BA in Psychology from the University of Massachusetts and her MSW from Simmons College. She is a Licensed Clinical Social Worker in the state of Massachusetts and a member of the National Association of Social Workers and the National Association of Professional Geriatric Care Managers.

Melissa A. Brady, LICSW, CSW-G, C-ASWCM
Professional Geriatric Care Manager

Melissa Brady, LICSW, CSW-G, C-ASWCM, is the Assistant Director of Social Work Services at the Jewish Rehabilitation Center where she has worked for the last eight years. She has also provided social work services for the Adult Day Health Program of Aviv Centers for Living. Melissa’s work experience includes medical social work and case management with elders and their families, with a focus on the dementia population.

Melissa also serves as a field supervisor for Bachelor and Master level Social Work students, and is an adjunct faculty member in the Social Work Program at Salem State College. Melissa has offered numerous workshops and educational programs locally and nationally regarding issues and programs surrounding elders and their families.

Melissa received her BS in Human Services with a concentration in older adults from Elmira College, and her MSW with concentration in older adults from Salem State College. She is a Massachusetts Licensed Independent Clinical Social Worker, and a member of the National Association of Professional Geriatric Care Managers. In addition, Melissa holds an Advanced Certification from National Association of Social Workers as a Certified Geriatric Social Worker.

Leslie C. Hazlett, LICSW, C-ASWCM
Professional Geriatric Care Manager

Leslie Hazlett, LICSW, C-ASWCM, founder of Options for Elders, a private geriatric care management company, has focused her work for the past 16 years primarily in the field of geriatrics. She has worked in outpatient and inpatient hospital settings as both a medical social worker and a case manager. Her work experiences include traumatic brain injury, chronic pain, obesity and acute medical rehabilitation.

Leslie received her BA in Education and Human Development from The George Washington University and her MSW from Boston University. She also completed post-graduate work in Case Management at Boston University. Leslie has been a Licensed Clinical Social Worker in the state of Massachusetts since 1993. She is a member of the National Association of Social Workers and the National Association of Professional Geriatric Care Managers.
