Should your loved one require additional skilled nursing or rehabilitative services after a hospitalization or rehabilitation stay, Aviv HomeCare will design a personal and comprehensive plan for recovery that specifically responds to medical, emotional, and personal needs.  Aviv Certified HomeCare’s services are provided in the following cities and towns: Swampscott, Marblehead, Lynn, Nahant, Salem, Winthrop, Chelsea, and Revere. Most services are covered by Medicare part A or other insurance.
